1. Applied by the person concerned in person:
   ID card and seal (or signature) of the applicant
2. Applied by interested party:
   In addition to ID card and seal of the applicant, please bring the original documents of interested relationship, and the remained copy or the certificate of kindred.
3. Applied by the consignee as a representative:
   • Power of attorney, ID card and seal (or signature) of the consignee
   • The power of attorney of applicant living abroad, which entrusts others to apply should be verified by our institution in abroad.
   • Certificate documents made in the mainland of China (for example, the certificates of interested relation, kindred, power of attorney, etc.) shall be verified by the Straits Exchange Foundation.
