Applicant：
- Parents, grandparents, household head (family head), common-law marriage, or adopters.
- “Children Welfare Institute” will be the representative applicant for the abandoned baby and the helpless children.
- Interested Party (if without the applicant of Item 1 or Item 2).
- Consignee
Certificate documents are required (original copies)：
- Household list for the registration of household; ID card and seal (or signature) of applicant.
- Certificate of birth, or certificate of birth investigation.
- If the applicant authorizes others to complete the procedures of registration, please submit the power of attorney, ID card and seal (or signature) of consignee.
- If the applicant was born in foreign country, please offer the certificate of permanent residence permit issued by Immigration Office for the initial registration of household after she/he comes back to Taiwan.
- If the parents are living within different household, please offer ID card or household list of the other party.
- If the applicant wants to take the surname of mother in accordance with the regulations of Civil Law, please submit the related copy of household register and agreement signed by parents.
