Applicant：
- Adopter
- If the adopter doesn't apply, the adoptee can be the applicant.
- Interested party (if without the applicant of Item 1 or Item 2).
- Consignee (the one who is permitted by the household registration office with appropriate reason).
Certificate documents are required (original copies)：
- Seal (or signature), ID card of applicant; and the household list and ID card of the adoptee (the ID card doesn't have to be submitted if it has not issued).
- Agreement of adoption agreed by the father, or the court verdict and the certificate of confirmed judgment, or the foster certificate of father.
- If the adoptee wants to take the surname of mother, the agreement of parents shall be submitted.
- Information of the parents' household.
