Application for the Issue of ID Card

Applicant:

  1. Applicant
  2. Statutory agent on behalf of minors under 14 year old
  3. Guardian
  4. Consignee

Certificate documents are required (original copies):

  1. Application for the initial issue of ID card: Seal (or signature) of the person concerned or statutory agent; ID card of statutory agent.
  2. Application of renewed ID card: Former ID card, and seal (or signature) of the applicant.
  3. Application of reissued ID card: Seal (or sign personally), and household list of applicant. (If the person concerned can provide credible papers with photo on them, which are clear to confirm the identity, such as passport, command of discharging from military service, certificate of graduation, etc., it can shorten the issuing time; however, the reissued certificate of the above-mentioned papers will not be accepted)
  4. Three 2-inch photos, uncovered, half-length, and front, of the person concerned within last two months, which must be thin monochrome or color photos of the same edition. (No wearing dark glasses)
  5. The person concerned who applies for reissued ID card should transact personally.
  6. The person who applies for renewed ID card can entrust others to transact with power of attorney. (Only for it can be identified the same person from the old certificate and new photo.)
  7. Minors, who are under 14 years old, shall be represented by their statutory agent to transact together. (If one of statutory agents cannot show up to transact together due to any reasons, he /she shall offer agreement to the other party to transact.)
  8. As for those who cannot show up to transact the application of reissued ID card due to special reasons, please inquire attendants.
