Applicant：
- Adopter
- Adoptee (if the adopter doesn't or can't apply).
- Interested party (if without the applicant of Item 1 or Item 2 mentioned above).
- Consignee (only for the one who is permitted by the household registration office with appropriate reason)
Certificate documents are required (original copies)：
- Agreement of termination adoption, or court verdict and certificate of confirmed judgment, or the verdict of termination adoption permitted by court, and certificate of confirmed certificate.
- Household list; ID cards, seals (or signatures) of adopter and adoptee.
- If the applicant wants to authorize others to complete the procedure of registration, please submit the power of attorney.
- The documents of termination adoption by the people in the mainland of China shall be verified by the Straits Exchange Foundation (including the power of attorney).
