Applicant:
- Spouse, relatives, head of household, common-law marriage, funeral undertaker, house or lane manager when the dead is deceased.
- The one who makes the announcement of death or the interested party.
- Interested party (if without the applicant of Item 1 mentioned above), and trustee.
Certificate documents are required (original copies):
- Diagnose certificate of death, or examination certificate of death, court verdict of death announcement.
- Household list and ID card of dead; ID card and seal (or signature) of applicant.
- If the dead is deceased in the mainland of China , the certificate of death shall be verified by the Straits Exchange Foundation.
- If the dead deceased in foreign country, the certificate shall be verified by the units in foreign countries; furthermore, the certificate shall be translated into Chinese and be verified by the Bureau of Consular Affairs of the Ministry of Foreign and the courts of justice.
- If the applicant cannot submit the certificate of death for the dead was deceased many years ago, the buried permit issued by hygiene authority is needed for the registration of death. Or submit the written record of reporting to police institute and the certificate of death signed by two witnesses who witnessed the death of party concerned on the spot after reporting to the police institute.
- ID card, seal (or signature), and power of attorney of consignee.
